Notary Forms

Please have a parent or legal guardian of the minor getting tattooed or pierced print out the notary form, and get it notarized before your tattoo or piercing session.

We require the parent or legal guardian to have a valid ID brought in to the studio with the notarized form to preform the tattoo or piercing.

The parent or legal guardian must be present while the minor is getting tattooed or pierced.
